fix-header.c (read_scan_file): Use cpp_fatal.

* fix-header.c (read_scan_file):  Use cpp_fatal.
Use renamed function names, and return protocols.
* fix-header.c (check_macro_names):  Fix struct parse_file->cpp_reader.
