(gen_reload): Renamed from gen_input_reload.

(emit_reload_insns): Delete SECONDARY_MEM_NEEDED case for input reloads that
can't happen and doesn't work.
Fix errors in use of reload_outxx insns.
Simplify output reload code by using gen_reload.
