lang-specs.h: Pass -MD, -MMD and -MG to cc1obj if configured with cpplib.

	* objc/lang-specs.h: Pass -MD, -MMD and -MG to cc1obj if configured with
	cpplib.
	* cpplib.c (cpp_start_read): If in_fname is not initialized, try to
	initialize it using fname.
