	* exp_disp.adb (Expand_Interface_Thunk): Do not generate thunk of
	eliminated primitives.
	(Make_DT): Avoid referencing eliminated primitives.
	(Register_Primitive): Do not register eliminated primitives in the
	dispatch table. Required to add this functionality when the program is
	compiled without static dispatch tables (-gnatd.t)
