Options

gcc.texi, [...]: Add index of command line options. 

	* doc/gcc.texi, doc/invoke.texi: Add index of command line
	options.  Use @opindex to index them rather than other indexing
	commands.  Use @var{...} instead of <...>.  Avoid spaces in names
	inside @var.  Don't use @kindex to index escape sequences in
	source code.
